Personal & Professional Referrals
We offer two different ways to join us here at the Chrysalis Centre. We accept self-referrals and professional referrals.
Professional Referrals
If you have either a client or patient that is aged 18 or over, identifies as female, and you feel that emotional support would help.
Self Referrals
If you are aged 18 or over, identify as female and require emotional support, we may be able to help you.
How to complete a referral
Please complete the referral form using the link below. Once the referral form has been received, we will contact your patient/client directly to complete their registration over the phone and talk to them about our services.
